In Pologne 2011 the breakaway one and everyone else went outside the time limit.

In the route du sud in 2007 or sooner I'm not sure, on the first stage, the breataway took 1h37 to the pack. The pack was on time to be disqualified, but of course they weren't, but all the team who were not in the breakaway had a penalty to pay for lack of combativeness... the race director was very very angry against the Protour team who did really not care about the race...

There was a youth race in Flanders where 1 rider put all other riders out of the race like this.
