I`ve been playing PCM 2012 for a few months now and am enjoying my Custom Team game greatly.
I am up to the middle of August (first season) and wondered If I download the Real Name ONLY Database, can I continue playing my Custom Team save or will I need to re-start again for the name changes to take effect?

Is it possible to go into the Data base Editor that comes with the game and just edit a few names (Carbonara !) save ,and I can carry on with my Custom Save Game?

Yes, that's possible. Use Lachi's editor, open your save-game(not your DB, but your save-game )
Then just go to Dyn_Cyclists, and change the names(Surname, name, and combination(I don't know how it's called, just the one that says F.Carbonara))
You can do the same at Dyn_Team, and then the team names.
When you're done, extract(not save, extract) your save, preferable under another name. Then open that save in-game, and you're done
But why don't you install the PCMdaily DB in the first place?
It might even be possible to edit in all rider names from the Real Name DB, but a bit more tricky.
You'll need an editor that extracts the DB tables into Excel files, such as the Excel Editor of the Full Editor. Open the DYN_cyclist table both from your savegame (located in My Docs/PCM12/Users/username/Database/Solo/Official Release) and from the real name DB. I'm not sure if in August the young riders are already added in DYN_cyclist, so check the number of rows. If the number of rows from the table in your save is the same as the real name DB, then you can just copy the three name columns (last name, first name and firstlast name) from the real name DB to your save (make sure they are both ordered by IDs in order to not have mistakes).
If the young riders are already in, though, you'd need to 'filter' them out. One way to do is would be: create a new column in your save (just temporarily), and copy and paste the IDs from the real name DB there. Then select both columns and highlight the duplicated cells (that link is for Excel 07, but I believe there are other ways in other excels, e.g. this).
After that, you'd need to order the ID column by color (thus leaving the "pre-existing" riders at the top, and the new ones at the bottom), and then copy and paste the name columns. Again make sure that the IDs are in the same order in the real name DB and among the 'pre-existing' riders. Then delete the column you had created.
After either of the methods above you need to save the table and extract the .cdb back to the savegames directory. And there you go, names corrected (yes, it might look quite tricky, but well it's the way I can think of).
Lastly, as Jesleyh said, you should consider downloading a big DB even if you'll not be playing with it in this specific career. There'll be other improvements that you'll be able to enjoy at this career - such as correct jerseys, real sponsorkits, and race variants - and, if you'll be creating a career again at a future point, you'll already have a DB installed
