Each stage has a value called MO/HIL ratio which defines which of these stats is used when climbing. If the value is 0, basically only the hill stat is used when climbing, if it's 1, then only the mountain stat, if it's 0,5, it's 50%/50% and so on. You can see this opening the stage info with the Stage Viewer/Editor. Another way would be to see which riders are suffering more in the early climbs of the stage, those better in hill or in mountain, but this is not much precise. You can also have an idea by looking in the profile (if there are mostly mountains or hills) and the stage classification (usually a hilly will use more the hill stat than the mountain one, and the other way around).

Hmm... this is something I'm not an expert on, but at least I know that most of the time, both does count a bit.
Exception is a very steep hill less than 1 km, where there is no influence of the mountain skill, and some really long climbs, where there is no influence of the hill skill.

For example, if you have a cyclist with Mountain 74, and Hill 80. There is a small mountain ,with a climb of 3 km. The Hill 'weigh'is 60% and Mountain is 40 %
(80x6+74x4)/10 = 77,6. His skill for this mountain is 77,6.
%'s are just fictional, I dunno how much % it really is...

EDIT: Whoops, quite the same story is in the post above me...

Oh, and just to make clear: this value counts for the whole stage. So if you have a stage which starts with several short steep hills and then have 3 HC mountains, probably the value will be very close to 1, for the whole stage (including the hills), in which case you'll see a rider with 80 MO 70 HIL doing better than a rider with 80 HIL 70 MO in the beginning of the stage (the hills).

I read somewhere that in PCM12 this is real-time calculation. But I can not remember if it was just announced by Cyanide or impression of players(?) Looking at the profile can be misleading, I have lost mountain stage which really was hilly (Breschel wins). Other stage seemed to be easier (looking at the profile) so I choose puncheur as a leader and it turned out that he has no chance cause this was really 100% mountain stage...

There's no chance. It works as described by Luigi.
